How are renal disease, renal failure, and renal impairment defined in drug monographs?

Clinical Pharmacology uses the following three definitions to describe renal conditions as they relate to drug therapy:
- Renal Disease: Describes any disease involving the kidneys, regardless of the impact on renal function.
- Renal Impairment: Decreased renal function, specifically as it relates to the removal of waste and medications. Because many drugs require dosage adjustments or special monitoring based on kidney function estimations, a threshold of creatinine clearance less than 90 mL/min or eGFR less than 90 mL/min/1.73m2 is used for the term “renal impairment”.
- Renal Failure: Severely compromised renal function correlating to a creatinine clearance of less than 10 mL/min or eGFR less than 10 mL/min/1.73m2.
Please note: for some drugs, two, or even all three terms may appear in the Contraindications/Precautions list in a drug monograph.
